With a couple of clicks in Prism, you can quickly and easily authorise a therapy review for these patients.

Prism Pharmacists will then perform therapy reviews for the patients you authorise, optimising their treatment and maximising your contract attainment.

What is Prism?
Prism is made up of two core components designed to work seamlessly with your organisation for maximum patient benefit: The Prism Tool, and the Prism Service.
PRISM – TECH
The Prism Tool is an easy to use Analytics product which segments and stratifies your patient population and highlights those patients where an intervention may be beneficial.
Leading-edge, but simple to deploy, we’ll activate up to 5 users per organisation to use the Prism platform. At least one should be a senior clinical member of the team so they can authorise therapy reviews where appropriate.
PRISM – SERVICE
The Prism Service is 2 days of Clinical Pharmacist time as standard. The Clinical Pharmacist, where authorised by you, will perform Therapy Reviews on your patient lists, optimise their treatment and medications in line with the latest NICE guidelines, and help pick up where these patients may be missing from registers.
